About Us

 

Colonial P.T.  is in the business of providing outpatient rehabilitative physical therapy services to the central NJ, Monmouth County area.  Services will include the treatment of injuries as prescribed by the referring physician, to include the application of modalities, joint mobilization techniques, controlling and normalizing flexibility, ROM, muscular strength, pain relief, sensation normalization and proprioception/balance training.  Services will be made available with a physician's referral to the population of Monmouth County. 

 

 

Colonial Physical Therapy is a limited liability corporation owned and operated by Thomas A. Hoffman and Ralph Desiderio.  Both are licensed physical therapists in the state of NJ.  

 

Colonial Physical Therapy has a beautiful 3,100 square foot facility located in “The Fountains” of Middletown. Across from the Middletown Post Office.

 

Who are we?

 

 

As part owner of Colonial Physical Therapy, Thomas A. Hoffman brings seven years of management experience to his position. Thomas holds a combined Bachelor of Science degree/Masters of Science Degree from The College Of Staten Island in Physical Therapy. Thomas' education and experience in the medical field come from his extensive training in physical rehabilitation and biomechanics as a major part of a Health Science degree. Thomas was a clinical manager of Kessler Physical Therapy and Rehabilitation from 2000 to 2002. Thomas brings valuable and trusted experience in the outpatient setting, having worked in this field for over twelve years.  

 

Ralph Desiderio is a licensed, practicing physical therapist for twelve years and co-owner of Colonial Physical Therapy. Ralph holds a combined Bachelor of Science/Masters of Science in Physical Therapy which he earned at The College Of Staten Island, as well as a Bachelor’s Degree in Physical Education from Manhattan College.


Both are classmates and friends since 1995 who have always dreamed of opening and managing their own physical therapy facility. This dream was realized in August of 2004 thanks to a special person “Uncle Marty”.
