Below is a list of medical insurance we take and/or we are in their network. If you do not see your insurance carrier, please contact us. We may still be able to accept your medical insurance as an out of network provider (most insurance companies have out of network benefits. We would just need to call and check what benefits these are).
As a courtesy to you, we will verify your benefits to ensure that you are well informed of your treatment options. Should there be a discrepancy on benefit details reported, we encourage you to also contact your carrier to be aware of your covered and non covered benefits.
You are ultimately responsible for knowing the details of your coverage (e.g., need for a physician's referral, pre-authorization, number of visits allowed, percent of coverage, deductibles, copayment amounts, etc.), which may determine the extent of your financial responsibility.
Fee for service is always welcome for those without insurance. We will do our best to arrange affordable payment for services received. You will find our fees to be reasonable and competitive. We also except credit card payment(s) for those individuals interested in this type of payment option.
All workers’ compensation and motor vehicle claims are accepted so long as you have an open claim and case number. Please have this available when you contact us.
